I have a patient whom has an open area on his coccyx, a flap was attempted, it dehisced and healed without closing completely now he has a slit 11x.5x 8.5, the skin is macerated. What product should I use to heal the dehisced area, fill in the depth, and clear up the already healed macerated area of the skin?

First we have to know why thre flap failed-most probably due to pressure and free from contamination from urine or faecal matter. So now it has break down,I suggest we tried to preserve as much of the flap if possible but if it is not possible maybe we should keep it clean and promote moist wound healing.
Thanks, Ang...I did just that and we went in this week for a re-closure. woundva to cleft of buttocks. Better bed.
He is overweight, and a paraplegic so that says it all. I've been using impregnated hydrogel to promote healing. Thinking I might try collagen, to get the collagen fiber connecting? thank you for your reply.
You should have a hyperbaric oxygen evaluation, faliled flap is a medicare approved indication for HBOT.
HBO is an option for failed flaps. Have you tried a KCI Wound VAC?
he is on a wound vac now..progressing well.. thanks all.
