Have you ever had a week where you feel like you can't win with wound care?  I mean one of those weeks where you get the perfect treatment going for a wound and another nurse comes along behind you and gets the order changed?  Or the night and weekend nurses who signed off on treatments, you know for a fact were not done?  Or what about treatments that were ordered that you know are inappropriate for the wound being treated? 

 

I personally like weekly wound care team reviews.  We recently started doing them at my Home Health agency and we have our WOCN present that reviews our pictures and we have a round table discussion about the best treatments and troubleshooting for wounds that are not healing.  Everyone gets to give input on things they have tried or would like to try and puts everyone on the same page.  To ensure a treatment gets done, I like having nurses date and initial their wound dressings.

When I was heading up the wound care team for a large home care agency we used a similar approach.  I found the team meetings to be invaluable!  The use of pictures was so important as it allowed everyone to get a better sense of what was going on with the patient.  I saw a tremendous improvement in outcomes with a substantial reduction in the cost of care.
