Wound to heel is unstageable would be Deep Tissue Injury.  Would treat it as DTI. Wound on achille tendon looks like hematoma unopened. I don't see any drainage coming from the wound.  Possible injury from pressure of boot or surface foot has been on. Would place foot up off bed offload pressure from foot.
